Final Fantasy Hacktics

How does fftpatcher patch data in an iso?  Does it extract the file, modify it, and reinsert it?  Does it edit the data in it's compressed form, in the iso?

I think it opens the iso directly because it won't patch if you open the iso in CDMage simultaneously.
